Arizona Issues Unemployment Debit Statements
Arizona issued debit statements for the first and second quarters of 2011 on September
17, 2011 to employers that use a third party tax filer and/or file electronically.
As reported in previous issues of the Flash Report, specifically issues 4, 5, 11 and
14, and on the TALX blog, employers in Arizona are being charged a Special Assessment
(SA) of 0.4% retroactive to the first quarter of 2011. The debit statements
detailing the amounts due for the first and second quarters of 2011 will be included
in the third quarter, 2011 blank quarterly contribution report (Form UC-018) mailing
for those employers that file on the paper form. Payment is due October 31,
2011 and can be made in conjunction with the third quarter payment or separately.
